ELIGIBILITY
Entries are open to male, amateur golfers, minimum age 50 as of October 10, 2008, who are bona fide residents of the State of Alabama and have a current GHIN number issued through the Alabama Golf Association.

Maximum allowable combined team handicap index is 16.8 strokes under the terms of the USGA Men's Handicap System. The AGA will be the final authority in determining whether or not a handicap that has been presented qualifies as an "approved USGA handicap".

Entries are subject to rejection, or withdrawal, at any time at the discretion of the AGA.

FORMAT
The Championship will be conducted at 54 holes of Four- Ball Stroke Play, 18 holes daily on Friday, Saturday and Sunday.

The field will be limited to 48 teams. In the event the Championship becomes over-subscribed, the final field will be determined by team handicap index. There will be a cut to the low 32 teams and ties following 36 holes of play.

In the event of a tie for the Championship there will be a "multi-hole" play-off commencing immediately following play (the holes involved to be determined by the AGA) with the Champion being determined by the lowest total of those holes. If a tie, or ties, still exists, the Championship will then be decided by "sudden death" following the same sequence of holes. After the Champion has been determined, all other ties not yet settled, including those involving teams participating in the playoff for the Championship, will be decided by matching scorecards of the final round in accordance with the USGA recommendations shown in Appendix I of the 2008-2009 Rules of Golf, How to Decide Ties,  page 124(c), except that if the tie still exists following this procedure, scores for the first 9 holes shall be treated in the same way until a winner is produced. Any remaining ties will be decided by matching cards hole-by-hole, beginning at the 18th hole.

RULES OF GOLF
Play will be governed by the 2008-2009 Rules of Golf as published by the USGA and by the Local Rules as established by the AGA. Questions will be settled by the AGA Rules Committee, whose decision shall be final.

CONDITIONS
The AGA Tournament Committee reserves the right to alter any of the conditions and schedules herein. The decision of the AGA in any matter shall be final.

ELECTRONIC ENTRY
1. To submit an entry (electronic entries only), simply click the register button at the bottom of this page and follow the step-by-step procedure.  All entries must be RECEIVED in the AGA office no later than 5:00 P.M., Thursday, September 25, 2008.

2. No entries will be accepted after 5:00 P.M., Thursday, September 25, 2008.

3. An entry fee in the amount of $280.00, payable by credit card, must accompany the entry application, $10.00 of which will be donated to the Alabama Turfgrass Research Foundation.  

4.  Upon successfully completing the entry procedure, an email confirmation will be automatically generated and sent to you at the email address you designate.

5. In the event the field has to be pared due to over-subscription (see Format above), applicants will be notified of the status of their entry as soon as practicable following the close of entries on September 25th.

6. Withdrawal: If a team withdraws before 5:00 P.M., Thursday, September 25, 2008, the entry fee will be refunded upon request, subject to a $15.00 service fee. No refunds will be made after 5:00 P.M., Thursday, September 25, 2008, unless AGA returns your entry for some reason, which could include paring back the field. All dates and time refer to date and time of receipt at the AGA office.

If the entry is rejected for any reason(s), you will be immediately advised of the reason(s), at which time you can correct the problem(s) and re-submit the entry form.

REGISTRATION
Players, and this includes both team members, MUST register IN PERSON prior to beginning play, or, if playing a practice round, prior to playing the practice round. Failure to do so may result in rejection of the player's entry. Inquire in the Golf Shop regarding the location of registration.

7:30 AM - 2nd Round, 18 holes.

7:30 AM - Final Round, 18 holes.

PRACTICE PRIVILEGES
Musgrove Country Club extends practice privileges to eligible players on Thursday, October 9, 2008. Starting times MUST be obtained from the golf shop (205.221.7902).

AWARDS

Awards will be presented to the low five teams posting 54 hole scores.

Ties will be decided as described in the "Format" section of this entry form pertaining to all ties except for the Championship.

In addition, the Champions will have their names engraved on a permanent trophy to keep in their possession for one year.

CADDIES & CARTS

No caddies are available at Musgrove Country Club. It is permissible to bring caddies and any player who does so is solely responsible for his/her conduct. Contestants are not required to have either caddie or cart and are permitted to carry their own bag.

Golf carts are allowed and are available to those contestants who have a valid driver's license.

Users must obey the cart operations rules established by the host club, to include the allowance of spectator carts.

For further information regarding carts, please refer to the AGA General Rules.

Please specify on the entry application if a cart is to be reserved.

DRESS CODE

The dress code at the championship site will be strictly enforced. On the golf course, jeans are NOT permitted and shorts MUST be "Bermuda" length, or mid-thigh. No shorts approximating "tennis" length will be tolerated and this includes cut-offs, tennis shorts, bathing suits, etc. Shirts must have collars. Any player not meeting these standards will be asked to remedy this situation prior to play.

If you have any doubts as to what dress will be accepted, please ask any AGA official prior to beginning play.

CONDITIONS AGREEMENT

We are members in good standing of the Member Clubs of the Alabama Golf Association at which the GHIN numbers indicated on the opposite side of this card are registered.

We agree that the Alabama Golf Association has the right to reject, or withdraw, any entry at any time.

We are amateur golfers and understand the "Rules of Amateur Status" as established by the United States Golf Association.

We, as participants in this Championship, assume full responsibility for all our acts, whether they be negligent or otherwise, and hold harmless the Alabama Golf Association and/or any club hosting this Championship for any injuries.
